ANNUAL ATHLETIC/CO-CURRICULAR TRANSPORTATION

In order to preserve safety and team camaraderie, CVUSD students are required to travel on busses, vans, or other transportation (when available) provided by the District to designated school sponsored activities, practices, and/or games/events.

Your continued support in this effort is very much appreciated.  In order to continue to deliver this valuable service to all participating students, we ask that you provide a contribution to help offset the expense.  The following are the proposed contribution rates:

1st Sport/Activity:   $125

2nd Sport/Activity:  $75

3rd Sport/Activity:  $50

Maximum Single Student Contribution:  $250

Maximum Family  Contribution:  $500

Thank you for your continued support!  It is our goal to make sure our students are transported to and from events, games, etc. through the safest mode of transportation.

*No student will be excluded from participation due to a lack of sufficient funds.

Although students are required to travel on buses, vans, or other transportation provided by the District to all school sponsored activities, practices and/or games/events, because of compelling needs, parents may need to transport their child or ward.

This form must be signed by the parent, student, and head coach

before submitting to the Athletics Department

24 hours in advance of the date of the contest.
